Cielo ClinicTM Improves Management of Chronic Conditions and Targets Quality Improvement ANN ARBOR, MI – Healthcare information technology (IT) company Cielo MedSolutions today launched Cielo Clinic – a chronic condition care management system – at the 29th Annual Family Medicine Research Conference held today at the University of Michigan. Cielo Clinic assists ambulatory care medical practices with the delivery of effective preventative care information and chronic disease management services. Based upon ClinfoTrackerTM software licensed from the University of Michigan Health System (UMHS) Department of Family Medicine, Cielo Clinic provides point-of-care prompt and reminding of services, proactive patient communications, and improved administration of clinical quality programs such as pay-for-performance and physician scorecarding. The affordable and simple-to-use toolset fits into a practice's existing workflow and accommodates either paper-based or electronic medical charting systems. UMHS uses Cielo Clinic to deliver over 140,000 patient encounter forms yearly and manage over 70,000 patients with 284,000 chronic conditions. The software's prompting feature led to a 400% increase in lead screening for pediatric Medicaid patients. Unlike electronic medical record (EMR) software, which typically focuses on billing data, Cielo Clinic collects, tracks, and reports on clinical data, which allows for better decision-making on an entire patient population. The software also generates reports that are useful for Health Plan Employer Data and Information Set (HEDISTM) score reporting and payor incentive programs focused on clinical quality measurements such as pay-for-performance. Based on web technology, Cielo Clinic is quick-to-implement and requires minimal on-site application maintenance. The software integrates with existing healthcare IT systems, including electronic medical record, scheduling, and billing systems. The low monthly subscription cost and short implementation time offer a quick return on investment no matter the size of the ambulatory practice.
