At the point-of-care, Cielo Clinic utilizes an encounter form for each patient's visit that reminds the provider about services due, documents problems and active medications, and records actions taken, including non-billable actions. The encounter form can be printed or completed using a real-time web interface and is customized to fit your practice's requirements. Cielo Clinic also captures vitals, patient lifestyle characteristics (such as obesity and smoking), symptom and non-billable diagnoses (such as cough) and over-the-counter medications such as aspirin for heart disease patients.
Make Office Visits More Efficient with Customized Patient Materials
To support activated and engaged patients, Cielo Clinic generates a variety of patient-facing materials. At the point of care, a patient can be provided a one-page health summary that documents key indicators over time and informs him or her of future needed services and tests. Customized patient questionnaires, such as a depression survey, can also be generated.
Easy Access to Patient Information
Cielo Clinic's clinical data repository gives you, from a single screen, the information you need to manage your patient. Documented problems, services delivered (and their values), encounter history, medications, allergies, guideline rules triggered and patient vitals are provided for historical lookup and analysis.

Cielo Clinic reaches out to patients who are not scheduled for a visit, but who are due for services such as lab tests or follow-up appointments. This sample letter is printed on the practice's letterhead and customized with the patient's information and needed services. Proactively reminding patients of necessary services helps increase patient satisfaction while at the same time increasing practice revenues.
Track Outstanding Orders and Referrals
Cielo Clinic generates call lists that identify patients who are due for services or patients with ordered-but-uncompleted tests. This example call list details patients who have been given requisitions for either diabetic eye exams or diabetic creatinine screenings, but who never had the service completed. Office staff and nurses can use such a list to contact patients and help ensure compliance.

Cielo Clinic generates reports for a variety performance measurement needs. The example report shown here documents the current state of the patient population in relation to colorectal cancer screenings. Where the patient population is not in compliance, this report tells you "why" they are not in compliance so as to allow you to take action to drive clinical quality improvement.

Manage to Pay-for-Performance Programs
Another example of Cielo Clinic's powerful data analysis capabilities, this report details a provider's compliance with their PQRI quality measurements. Similar reports can be generated for other payor programs and practice guideline measurement approaches.
E-Prescribe
Cielo Clinic's e-prescribing module allows providers to prescribe medications, synchronize medication lists and allow for two-way communication between the registry and the e-prescribing tool, allowing a provider, from one place, to manage both care needs and prescribing needs.
Actively Support Care Management Activities
Cielo Clinic supports activities driven by disease management coordinators, nurse navigators and disease management companies. This includes support of patient enrollment in individual care plans that have their own goals and longitudinal monitoring of needs and results. Self-development of assessment forms allows care managers to develop specific questionnaires for each care plan, generate these forms at any time for customer completion and capture the results of these questionnaires in the database.
