百里酚聯合他唑巴坦對β-內酰胺類抗生素抗MRSA活性的影響

摘要:耐甲氧西林金黃色葡萄球菌(methicillin-resistant Staphylococcus aureus,MRSA)在社區和醫院廣泛流行,引起多種重癥感染,嚴重危害人類健康。廣泛的耐藥性導致MRSA感染治療困難,具有極高的死亡率。MRSA對β-內酰胺類抗生素普遍不敏感,通過藥物組合的方式增加MRSA對β-內酰胺類抗生素的敏感性具有重要意義。本研究發現百里酚在明顯抑制葡萄球菌黃素產量的濃度下,能將青霉素、頭孢菌素和碳青霉烯類抗生素的抗MRSA活性提高4-16倍,且百里酚與阿莫西林還在抑制生物膜形成、清除成熟生物膜和膜內細菌方面體現出良好的協同效應,提示這種組合具有一定的抗耐藥性。進一步聯用β-內酰胺酶抑制劑他唑巴坦能將百里酚/β-內酰胺類抗生素的抗菌活性再提升4-8倍,提示百里酚與復方β-內酰胺類抗生素的聯用在抗MRSA感染中具有重要的應用前景。通過進一步機制探討和組合優化,有望為抗MRSA感染提供新的治療思路。

Effect of thymol in combination with tazobactam on the anti-MRSA activity of β-lactam antibiotics
Abstract:Methicillin-resistant Staphylococcus aureus (MRSA) is widely prevalent in communities and hospitals, causing a variety of severe infections which seriously threaten human health. Moreover, the widely resistance of MRSA made MRSA-associated severe infections difficult to treat and with high mortality rate. MRSA is generally insensitive to β-lactam antibiotics, so increasing the sensitivity of MRSA to β-lactam antibiotics through drug combinations is of great significance. In this study, thymol was found to increase the anti-MRSA activity of penicillin, cephalosporin and carbapenem antibiotics by 4-16 times at concentrations that significantly inhibited the production of staphyloxanthin, and thymol and amoxicillin also showed good synergistic effects in inhibiting biofilm formation and clearing mature biofilms and the bacteria in the biofilms, which indicated that this kind of combination has certain anti-drug resistance. Further combination of tazobactam, a β-lactamase inhibitor, increased the antibacterial activity of thymol/β-lactam antibiotics by 4-8 times, suggesting that the combination of thymol and compounded β-lactam antibiotics has important potential in the fight against MRSA infection. By further mechanism exploration and combination optimization, it is expected to provide new therapeutic ideas for anti-MRSA infection.
