Through this form, providers are given a snapshot of each patient's needed services and existing problems. Providers simply document care actions taken on this form and a practice staff member enters the information into Cielo Clinic in a matter of seconds. Through this interaction, a practice is building an all-problem, all-patient registry of clinical information and can use the patient outreach tools to reach patients due for services but not scheduled for a visit. The burden of quality reporting can be eased by eliminating the need for costly chart pulls.
